Updating Your Name for your CACVT Account

Has your legal name changed, or does the legal name listed in your CACVT account need to be corrected?

You may use the Name Change Request Form to request an update to the legal name listed in your CACVT account. This form may be used for legal name changes due to marriage, divorce, court order, or another legal process. It may also be used for corrections to the legal name currently listed in your account, such as a misspelling or missing middle name.

Your CACVT credential and account record should reflect your full legal name and should match your Colorado DORA registration and other professional credentials, when applicable.

Please read all the following information before submitting your name change request.

– Required Documents for Legal Name Changes –

If your legal name has changed, you must upload legal documentation that supports the requested name change.

Accepted legal name change documents include:

Marriage license or marriage certificate*
Divorce decree
Court order or legal name change order

* If your post-marriage name change includes a change that is not fully reflected on your marriage certificate, additional documentation may be needed. This may apply if you also changed your middle name, use a hyphenated surname, use a combined surname, or are requesting another legal name format not clearly shown on the marriage document.

– Document Upload Requirements –

Before submitting your request, please make sure your uploaded document meets the following requirements:

All pages of the document are included.
The full page is visible, including all four corners of each page.
The applicant’s name is visible and readable.
The document is clear, complete, and not cropped, blurry, password protected, or incomplete.
The file is uploaded as a PDF, JPG, JPEG, or PNG.

If the submitted documentation does not clearly support the requested update, CACVT will contact you with next steps.

– How to Save Your Documents –

The Name Change Request Form accepts one document upload. If your document has multiple pages, all pages must be included in one file before uploading.

You may combine multiple pages into one file using one of the options below.

Using a Smartphone

Most smartphones allow you to scan multiple pages into one PDF. Before uploading, review the PDF to confirm all pages are included, readable, and in the correct order.

iPhone or iPad Users – Open the Notes app, create a new note, tap the camera icon, and select “Scan Documents.” Scan each page, then save the completed scan as one PDF.
Android Users – Open the Google Drive app or another scanning app, tap the plus sign or scan option, scan each page, and save the completed scan as one PDF.

Using a Scanner

Use your scanner’s document feeder or flatbed scanning option to scan each page of the document. Before uploading, open the saved file to confirm all pages are included and readable.

Using File Editing Software

If you already have separate image or PDF files, you may combine them using native file tools or PDF editing software. Before uploading, review the final file to confirm all pages are included, readable, and in the correct order.
